Your medical records will continue to be maintained securely and confidentially, in full compliance with New York State law and the Health Insurance Portability and Accountability Act of 1996 (HIPAA).
No action is required if you wish to continue your care with MAF Medical, P.C. (doing business as Ever/Body). Ever/Body will serve as the custodian of your existing medical record for purposes of ongoing treatment and continuity of care.
If you wish to see a different provider, we will—upon receiving a written request—send a copy of your medical record to that provider or provide a copy directly to you.
If you prefer that MAF Medical, P.C. (dba Ever/Body) not access your existing Telo Labs record for treatment purposes, please notify us by emailing hello@telolabs.com within 30 days, so we can document your preference while ensuring you continue to have access to your records.
